Recently in Haverford Pennsylvania a 78 year old woman suffering from dementia and living in a high end senior facility alleged that she was being abused both physically and mentally by staff. Her family members took this seriously and sought assistance with the administration at the facility called Quadrangle. The facility administration investigated and determined that the allegation was unfounded. However the family did not discount their loved ones claims and they hid a camera in the room. The footage from the video showed at least three different staff members abusing the woman. The abuse included hitting, slapping, pushing and acts of humiliation that included making the woman strip to the waist and stand topless while the staff members taunted her. There have been three arrests. Charges have been pressed against 22 year Samirah Traynham, 21 year old Tyrina Griffin and 19 year old Ayesha Muhammad. Charges include aggravated assault, criminal conspiracy, harassment, and neglect of a care-dependent person. District Attorney G. Michael Green described the actions of the three women as a 'humiliating' assault.
The Quadrangle is operated by Sunrise Assisted Living and has been known to describe itself in advertisements as a 'beautifully designed community', and goes further stating that it offers extraordinary opportunities for continued learning and exploration.
The state of Pennsylvania requires facilities such as the Quadrangle to be licensed. They are subject to inspections and there are complaint procedures. The state keeps files on every licensed facility and these are public record. Facilities are licensed according to type by various agencies in the state. The PA Department of Aging licenses Adult Day Service Centers and certifies Domiciliary Care Homes. The Department of Insurance licenses Continuing Care Retirement Communities. Community/Group Homes are licensed by the PA Department of Public Welfare. Whatever type of facility you use or are considering using, it is best to seek the history of complaints and the result of inspections through the licensing agency.

You may have to go through a few steps. You can begin with a phone reverse look up. Then, once you have a name and/or address, you have the information you need to look further. The sources are not always accurate and are sometimes with outdated information, but it is free to look up unless you’re using private companies.
