Application for Mississippi Driver's License

Certification of Diabetic

Consent to Release Records

Instructions for 17 and older Application
Use this application to apply for one of the following: an Operator's License, a Learner's Permit (17 or over), a Commercial Driver License (CDL), a Class "D" Commercial License (Non-CDL) or a Non-Driver's Identification Card. This application may also be used to change your name and/or address on you driver license and/or voter registration files.
Proof of Domicile
Effective July 1, 1999, any applicant for an original license who is over eighteen (18) years of age must show proof of domicile in this state to receive a license.
School Attendance Form
Certification of School Attendance Status - In accordance with Section 63-1-10, Mississippi Code of 1972, any applicant for a driver license under eighteen years of age must submit with the application documentation from the appropriate authority that the applicant is in compliance with Section 63-1-9(g), Mississippi Code of 1972, as amended.
