Pikeville is a home rule-class city and the county seat of Pike County, Kentucky, located in the heart of the Appalachian coal country of eastern Kentucky. With a population at the 2020 census and approximately 7,490 residents in recent estimates, Pikeville is the major urban center and governmental hub for Pike County, which with a population to 58,669 people is one of eastern Kentucky's most populous counties despite its remote mountain setting. The city takes its name from explorer and military officer Zebulon Pike of Lewis and Clark era fame, and was formally established as the seat of Pike County in 1824, formed from land owned by early settler Elijah Adkins. Pike County itself was named in honor of Zebulon Pike, a War of 1812 veteran who was killed in action during that conflict.Pikeville and Pike County sit at the confluence of the Big Sandy River's Levisa Fork and Elkhorn Creek, in a region of dramatic mountain topography that has both isolated and defined eastern Kentucky communities. The Big Sandy River valley served as an important transportation corridor long before the railroad arrived, and the establishment of rail service in the late 19th century opened Pike County's vast coal reserves to large-scale commercial extraction. The county became one of the most productive coal-producing regions in the United States, generating enormous wealth that transformed the physical scene through extensive mining operations while creating complex social and economic patterns that persist to this day. Among Pikeville's most remarkable physical features is the Pikeville Cut-Through, an engineering project completed in 1987 that relocated a mountain, the Levisa Fork River, and U. Highway 23 to create level land for development in the narrow river valley. The project required moving approximately 18 million cubic yards of earth and was one of the largest earth-moving projects in North American history, fundamentally transforming the city's geography and creating approximately 600 acres of new level land for economic development. The university's health professions programs help address the chronic shortage of healthcare providers in rural Appalachia.
